The Final Report for CCCFF is submitted after the expenses are completed and reviewed in the GMS, approved by the CCCFF Program Grant Manager, and the final reimbursement has been submitted.

Instructions on How to Complete CCCFF Final Report:
Navigate to the Custom Form Grid
Step 1: Navigate to your Grant - Grant Management (left navigation) > Grants > [Select your Grant]
Search Bar
If there are enough custom forms added to your award, you will have a 'Search Bar' at the top of your Custom tab.
Use this to filter down the forms that appear in the dropdown by entering in the Custom Form name you are looking for.
Creating and Submitting a Custom Form
For the 'Project Completion Documentation' sections, you only need to complete the portion related to your project (e.g., if you had a Construction Project, please complete the Construction section; if you had a Planning Project, please complete the Planning section).
- Documentation Requirements for Construction Projects: Attach documentation identifying that the project is complete (e.g., certificate of occupancy, certificate of substantial completion) and project photographs. In addition, upload a letter from the Chief Elected Official or Meeting Minutes certifying the project is complete as designed and contracted in accordance with the CCCFF contract. Where the project is incomplete, letter or minutes should include narrative that clearly indicates estimated project completion date and project status.
- Documentation Requirements for Planning Projects: Attach a copy of the final planning document. Additionally, upload a letter from the Chief Elected Official or Meeting Minutes certifying the project is complete as designed and contracted in accordance with the CCCFF contract.
