What are Custom Forms used for?
Custom Forms on Awards are used to collect both informative and performance reporting.
- Informative (e.g. ACH information for Payment Requests)
- Reporting (e.g. Annual report for project's progress)
Viewing Available Forms
To view available forms:
Custom Form Grid
After selecting the Custom Form, you will be taken to its Custom Form Grid.
A row will appear for each form that has been created. If the grid is blank, no forms have been created.
NOTE: In some cases the Custom Form just needs to be created once, and any edits should just be done to the 1 form. In other cases, the Custom Form will need to be created for each instance. Read the Custom Form instructions on this guidance.
From this Custom Form Grid, you are able to view previously created forms, edit forms that have not been submitted, edit forms that have been reopened, and create new forms.
To view previously created forms, click the row's green hyperlink field.
